Documents Required for Blood Bank License Renewal in Assam
- Declaration/Undertaking from the Head of Department (HOD).
- Key-Plan of the Building showing its relation to nearby/adjoining roads, buildings, plots, etc.
- Layout Plan (minimum 100 square meters) of the blood bank premises, clearly indicating the dimensions and positions of various sections, doors, windows, etc.
- Attested Photocopies of the following documents for the competent technical staff employed by the blood bank on a regular basis:
- Certificates of qualification and experience
- Bio-data
- Appointment letter
- Letter of approval
- Joining report
- Registered Nurse, Medical Officer, and Technicians
- List of Machinery and Equipment installed for blood collection/processing, providing full details, such as:
- Make
- Capacity
- Material
- Whether automatic or manual, etc.
- List of Laboratory Equipment/Apparatus, reagents, and reference books, duly signed by an authorized person/medical officer.
- List of Human Blood Components intended to be processed and the method of their preparation, duly signed by an authorized person/medical officer.
- Draft Specimen Labels for whole human blood/blood components intended for collection/processing.
- Standard Operating Procedures (SOPs) for various blood banking procedures, duly signed by an authorized person/medical officer.
- Undertaking from the blood bank that it will not draw blood from paid or professional donors.
- No Objection Certificate (NOC) from the State Blood Transfusion Council, Assam.
- Memorandum of Association/Constitution of the firm.
- Documents Relating to Ownership or Tenancy of the premises.
Application Process
Step 1: Registration
- Visit National Government Services Portal or State Health Department website.
- Fill out application form.
Step 2: Document Submission
- Upload scanned copies of required documents.
- Pay renewal fee
Step 3: Verification
- Wait for verification and processing.
Step 4: License Renewal
- Receive renewed Blood Bank License.
Fees and Timeline
- Application Fee: License fee of Rs. 6,000, plus inspection fee of Rs. 1,500 (Total: Rs. 7,500).
